Due to the COVID-19 outbreak, unfortunately at this time we are not accepting any new employer engagements until further notice. We hope to provide additional information regarding our 2021 plan in the coming months.


Men age 39 and under

Women age 39 and under

Men age 40-49

  • Comprehensive physical examination

  • Urinalysis
  • PSA — Prostate screen (age 45 and older)
  • Cardiac treadmill stress test (every three years)
  • Carotid intima-media thickness test (every three years)

Women age 40-49

  • Comprehensive physical examination
  • Pap smear
  • Mammogram
  • Blood lab studies
  • Thyroid stimulating hormone
  • Urinalysis
  • Cardiac treadmill stress test (every three years)
  • Carotid intima-media thickness test (every three years)

Men age 50 and over

  • Comprehensive physical examination

  • PSA — Prostate screen
  • Cardiac treadmill stress test (every three years)
  • Carotid intima-media thickness test (every three years) 
  • Aortic ultrasound — age 60 and older (every five years)

Women age 50 and over

  • Thyroid stimulating hormone
  • Urinalysis
  • Cardiac treadmill stress test (every three years)
  • Carotid intima-media thickness test (every three years)
  • Dexa-bone density (every five years)

 

For men and women age 50 and over: a colonoscopy is recommended starting at 50 years and then every five to 10 years, based on results. This is a separate day of service and may be covered by your employer plan.
