When you are discharged from the hospital there can be many changes made to your home medicines. These changes are very important and can help prevent a re-admission to the hospital. If you have questions about your discharge medicine list, contact your doctor's office to review these changes.

Planning for Your Medications After Hospitalization

Your doctor has written a prescription for the medication you will need once you leave the hospital. We suggest that you, a family member, or a friend fill the prescription(s) at the pharmacy you most frequently use. Your pharmacy will have a record of the other medication you are taking and can advise you on drug interactions, track refills, and provide other helpful information. Please be sure that the person picking up your medications has your insurance card and is prepared to pay for the prescription or any co-pay fees.

If you have questions or problems getting your prescription(s) filled, please contact your doctor's office.
